Required Volunteer Form

Little League Baseball, Incorporated Regulations require that all chartered Little League programs conduct an annual background check on all persons and are required to complete a “Little League Official Volunteer Application”. It is required that all of the following persons must annually submit a fully completed “Little League Official Volunteer Application” to the local league president prior to the applicant assuming his/her duties for the current season: managers, coaches, Board of Directors members and any other persons, volunteers or hired workers who provide regular service to the league and/or have repetitive access to or contact with players or teams.

The form must be downloaded printed and filled out and then submitted to the president of the league.  
Proof of Identity is also required, attach a photocopy of a government issued ID.

All coaches and managers must also submit a HeadsUp online Concussion training certificate.
